Not all clinics will perform the same duties or tasks, and employees will have specific tasks assigned as directly associated with their clinic assignment.)Under the supervision of the Operations Manager, the Community Health Worker (CHW) is responsible for assisting patients in navigating and accessing community services and adopt healthy behaviors. Assists clients to enroll in health insurance and health plans. As the frontline public health worker, serves as liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Conduct outreach in the community that promote, maintain, and improve individual and community health and assist with interpreting services. Provide information on available resources, provide social support and informal counseling, and advocate for individuals and community health needs. May perform HIV/STI testing, prep navigation, medication assistant treatment outreach, and linkage to care. Perform and document health assessments, obtain appropriate consent to collect data to help identify community health needs. Essential Functions Assess and coordinate community outreach to ensure patients are enrolled in appropriate programs and or services. Performs crises intervention and determines what resources are needed to promote health management and medical care adherence.Determine eligibility to programs and coordinates support services to address unmet needs. (i.e. shelter, transportation, insurance, finances, food etc.) Initiates referral to programs within the health department or other agencies in the community as needed. Provides necessary follow-up, tracking, and data collection to assure continues enrollment.Provide health education, assessments for social determinants of health and barriers, motivational coaching and social support to obtain care/services and promote risk reduction, prevention and healthy behaviors.Access word processing and other software programs to format documents, spreadsheets, training materials and other required documents. Enters data and produces various reports as needed or requested.Partner with community representatives and attend meetings and health care fairs to understand community needs assessments and provide information to community members. Identify the particular health care needs of individuals in a community to increase healthy lifestyles.As a Certified Community Health Worker, provide consumers with information about health coverage options, answer questions, and assist enrollment coverage, such as Marketplace plans, Medicaid, MI Bridges through MDHHS. May assist in training, educating and the onboarding process of CHW's within the community health centers.Maintain CHW communication/documentation within the EHR Patient Management System in accordance to the electronic health policy, and accreditation standards to ensure accurate documentation.Collaborate within an interdisciplinary team to service the clients/patients and process recommendations, assist in medication assistance program, psychosocial support service, may perform screening and assessment of social needs which could lead to care coordination, case management, and system navigation.Performs outreach activities which includes rapid-HIV testing for target populations. Assesses clients during face-to-face interviews for their risk of contracting HIV and other sexually transmitted infections and records relevant information in client charts and disease control forms.Helps clients effectively communicate with health care providers, provide informal counseling and support and follow-up procedures.Participates in specific health department/health centers programs to community groups, schools, professionals, and other interested people. Promotes and conducts local activities, assist with planning, coordination and implementation of programs, community workshops and events.Make referrals for community resources with follow-up coordination for identified social needs or barriers to care or adherence to treatment plans. Employment QualificationsEducation: A minimum of a High School diploma or equivalent is required.Experience: Experience with social services or public health system is preferred.Other Requirements: Certified Application Counselor or will acquire within six months of hire.
