Job Summary
Our client is seeking a Community Health Worker responsible for bridging the gap between the community and health services. The role involves promoting health practices, ensuring access to healthcare resources, and advocating for community health needs.
What You'll Do
- Conduct outreach in the community to educate and enroll patients in CalAIM
- Manage a caseload of 25–50 clients, including assessments and referrals
- Provide housing navigation and case management support as needed
- Set up outreach tables at clinics, hospitals, and community events
- Build relationships with local facilities and community partners
Compensation & Perks
- $25–$30/hour
- Performance bonus: $2,500 for every 50 enrollments in a single month
- Mileage reimbursement
- Training, marketing materials, and equipment provided
What We're Looking For
- Outreach or enrollment experience, ideally with CalAIM or similar programs
- Go-getter mindset with ability to work independently in the field
- Valid driver's license and comfort traveling to different locations
- CHW Certificate preferred (strong plus)
- Education: Bachelor's in Social Work, Psychology, or Sociology + 2 years' experience OR 4–5 years' direct experience in lieu of degree
