The Community Health Worker 1 provides health services in needs assessments, referral services, client education, documentation, and diversity education. Essential Functions of the Role. Assists clients in accessing services and increasing health knowledge and self-sustainment. Learns to use tools and services like outreach, patient navigation, and community health education. Assists with referrals and linkage to follow-up services in the community and the Organization. Assists with needs assessments and the development of patient plans. Conducts culturally appropriate skills-building self-management education sessions on various topics for groups and individuals. Builds knowledge of the community's ethnicity, language, socio-economic status, and life experiences. Learns and assists in teaching basic concepts of health promotion, disease prevention, and self-management. Obtains basic vital signs, measures height and weight, conducts point of care testing.
