Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We are looking for a Community Health Outreach Worker to join our Mobile Medical Team! Central City Concern's Mobile Medical Team provides medical care outside of traditional settings. The MMT partners with existing outreach teams at CCC and works to enhance our spectrum of street-based outreach support and care. The MMT will provide outreach and medical care directly to people living in sanctions and unsanctioned camps.
