THIS ROLE REQUIRES A CARAnnual salary range: $82,800 - $95,220 $3,000 Sign-on Bonus! Schedule: Monday - Friday 8am-4:30pm The Community Health Nurse will support Residential Programs and the clients that live in these settings at the various residential sites across the north side of Chicago by being present on site and engaging with clients in their homes. They will support in the assessment and transition process of clients into and out of Trilogy's residential settings including group homes, PSH, and Cluster sites. The Community Health Nurse will support clients to maintain their health while living in community settings. They will provide supervision and consultation on medical illness, self-management, and medication monitoring, training, and medication reconciliation services. They will collaborate with the treatment team, providers, and others on client plans of care to ensure quality standards are met and support with medical visits in the community. The Community Health Nurse will provide strengths-based, client-centered, and trauma-informed services to clients with severe mental illness and co-occurring substance use disorders utilizing a harm reduction approach. RESPONSIBILITIES
  • Support the residential team conducting integrated health assessments of clients' health and develop medical treatment plan goals for the client to support in their recovery and management of their health conditions.
  • Educate the behavioral health residential teams on client's medical issues and provide care coordination services by communicating with physicians, specialists, pharmacies, home health providers, and inpatient/outpatient providers.
  • Coordinate and facilitate client's needs including hospital discharges and admissions and follow up, outpatient specialty appointments and procedures, etc.
  • Complete medication reconciliation process with residential programs to support in ensuring that clients are taking the correct medications and have access to all medications prescribed.
  • Provide psychoeducation, medication training and monitoring to clients according to Trilogy policy.
  • Visit clients in their homes and support with transporting clients to medical appointments in the community and attending appointments to support health management and continuity of care.
  • Address health needs of clients with serious mental illness and substance use disorders; i.e. administer specified medications, including long-acting injectable antipsychotics, obtain vital signs, point-of-care testing (POCT), etc.
  • Document in electronic medical records (EMR) according to organizational policies for assessments, injections, and progress notes in a timely manner.
  • Attend organization wide nursing meetings as scheduled and supervision meetings with both supervisors and Nurse Manager.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and or the need for stabilization through hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Provide individual-based motivational treatment to clients to assist them in their recovery from mental illness.
  • Communicate effectively within the team model for a multi-disciplinary approach to client care.
  • Participate in meetings, committees, and work groups as needed.
  • Potential for occasional phone availability off hours or on weekends to answer questions from staff on the team about specific clients that the nurse has a direct working relationship with to help give direction
  • Perform other related duties and/or projects as assigned.
QUALIFICATIONS
  • Associate degree in Nursing required; Bachelor's/Master's preferred
  • Current Registered Nurse license in the State of Illinois.
  • Active BLS certification required
  • Minimum two years' experience working with people with mental health and/or substance use diagnoses
  • Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
  • Experience working in residential settings preferred
  • Exceptional customer service skills and ability to interact with a diverse group of internal/external stakeholders
  • Psychiatric nurse in an inpatient or outpatient setting, community or public health nurse, addiction specialist nurse, and/or nurse care manager experience preferred
