Community Health Navigator - AmeriHealth Caritas Family of Companies : Job Details

Work Arrangement. This role is remote however candidates need to live in New Castle county in DE and be able to travel anywhere within that county. Must have a valid driver's license, valid/current auto insurance, and be willing to use personal vehicle. All travel for work is reimbursed. Member Outreach:Establishes connection and acts as the primary ACFC contact to a targeted member population, including some high-risk members, through home visits and outreach calls. Exercises judgment and discretion in coordinating care by handling basic follow-up tasks and collaborating with AmeriHealth Caritas care management teams, community-based healthcare providers, and other organizations. Informs members through phone calls or home visits of the required actions to fulfill HEDIS, EPSDT, and Plan requirements and other referred outreach and encourages them to comply.
