DescriptionSummary This position is responsible for assisting with maintaining a continuous dialogue with the community, assessing the community's needs, and conducting outreach for community health. Specifically, this position will assist in planning, designing, monitoring, implementing, and evaluating programs to educate the community and promote individual and community health. Will provide information, repond to queries, and engage with community members on the County's web pages and social media accounts. Assist with program-specific health promotion and marketing of services. May provide social support and advocate for individuals' and community health needs. May collect data to help identify community health needs with a focus on holistic health (e.g., fitness, nutrition, physical health, mental/emotional health etc.). This position is under the direct supervision of the Health Department Director and/or his/her designee. Definition and Examples of Work70% This position works with the Health Department administration to build and maintain continuous community engagement. Specifically, this position will work closely with supervisors to:
  • Identify or contact members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or pregnant women as identified through grant and program priorities.
  • Contact clients, community leaders and community groups in person, by phone, in writing, and via social media to promote healthy behaviors and advocate for appropriate healthcare
  • Map community health resources and link community members/groups to available/needed health services and resources.
  • Organize and attend community meetings or health fairs to understand community issues, build relationships with community members.
  • Manage Social Media accounts; grow outreach and marketing with the use of various social media platforms.
  • Advise clients or community groups on all available services provided at the Finney County Health Department.
  • Lead and champion marketing, communications and advertising for all Health Department programs and services.
  • Organize and coordinate educational seminars and events on high-risk targeted topics for staff as well as the community.
30% This position works closely with supervisory staff to identify community needs and develop resources for the community. Specifically, this position will:
  • Research and gather resources and develop appropriate infographics directed at specific audiences on various health-related issues.
  • Identify available sources of data, identify gaps in data, and develop methodology to collect data that helps establish community needs and priorities
  • Gather and analyze data to determine the need for various programs as well as measure their success. Track and evaluate all offerings, compile reports as needed by County and Grant administration. Evaluation of programs includes degrees of enrollment, engagement, completion, changes in knowledge, risks, satisfaction, and cost-benefit analyses among other methods.
  • Provide recommendations for improvement based on reported data that will make the greatest impact on participants.
  • Keep abreast of current national and state priorities and resources.
  • Develop a network of resources and contacts to use for Finney County as appropriate. Explore ideas and opportunities to personalize programs to increase community participation.
  • Work to effectively integrate Health Department services and programs with other resources available in the community to deliver a seamless, unified program to impact the health of Finney County.
  • Maintain confidentiality of all information and follow internal HIPAA policies and procedures.
  • Respond to change productively and complete all other tasks as assigned. Attend meetings/trainings as required.
Additional duties may be assigned as business needs require. Requirements
  • Excellent verbal and written English language skills, experience in report writing and presentation
  • Excellent interpersonal and customer service skills
  • Proficient in Microsoft basic computer systems and software (e.g., email, timekeeping software, work order inputs)
  • Ability to maintain confidentiality of information.
  • Ability to maintain accurate records of service and activities.
  • Knowledge of referral systems or having the ability to develop referral systems in the community.
  • Project management experience
  • Marketing knowledge and abilities to help design communication tools. Knowledge of graphic design software preferred.
  • Knowledge of a broad range of community health, topics and a commitment to community wellness and the promotion of life learning
  • Ability to establish relationships with community leaders, community, and staff members to promote participation in sponsored programs, and to motivate them to set and reach health goals
  • Ability to display professionalism when dealing with clients, community leaders and outside vendors
  • Ability to maintain confidential documents and information and keep abreast of HIPAA rules and regulations
  • Ability to present self as a forward thinker, energetic, motivated, creative, organized, relationship builder, and a leader
  • Presentation skills and ability to teach

  • Successful experience in the areas of needs assessment, program planning and development, marketing and promotion, implementation and delivery, and evaluation of programs.
  • Ability to obtain CPR and First Aid certifications
