Community Health Advocate

The Community Health Advocate will be responsible for helping patients and their support team to navigate and access community services, other resources, and adopt healthy behaviors. The Advocate supports primary care providers and clinic staff through an integrated approach to care management and community outreach. The Advocate will provide social support, informal counseling, and advocate for the health needs of individuals and their community. Community outreach such as home visits and visits to community agencies may be necessary. Essential Functions include:

  • Establishing trusting relationships with patients and their support team while providing general support and encouragement.
  • Providing referrals for services to community agencies as appropriate.
  • Helping patients connect with transportation resources and giving appointment reminders in special circumstances.
  • Transporting patients is strictly prohibited.
  • Exhibiting excellent working relations with patients, visitors, and staff, effectively communicating the mission of CHI Saint Joseph Health and the Robert Wood Johnson Foundation.
  • Working collaboratively with the health care team. Establishing positive communications and partnerships with providers, services, the patient, and the team.
  • Continuously expanding knowledge and understanding of community resources, services, and programs which could be beneficial to patients being navigated.
  • Acting as a community advocate between the patient/family and community service agencies.
  • Attending regular staff meetings, trainings, and other meetings as requested.

Qualifications:

  • Required Education: High school diploma or GED
  • Required License/Certifications: KY Driver's License
