Salary : $45,448.00 - $53,768.00 Annually Location : Dayton, OH Job Type: Full-time Regular Job Number: PH09251 Department: Public Health Dayton & Montgomery County Opening Date: 09/02/2025 Internal Only: No Position Overview Under the direction of the Chronic Disease Prevention Senior Manager, the Community Health Advocate I/II is responsible for empowering individuals and communities to manage and prevent chronic illness, specifically tobacco prevention and cessation. They act as a liaison between residents and the complex healthcare system, offering direct support to community and clients while also advocating for systemic improvements. Summary of Job Duties Responsibilities include but not limited to:
  • Planning and developing community health-related programs including curriculum, program material, educational pamphlets, and related literature; Evaluating the effectiveness of educational efforts.
  • Collecting data and assessing outcomes related to the Local Tobacco Prevention and Cessation Grant.
  • Complying with grant requirements, including periodic reporting.
  • Developing and supporting local partnerships to broaden the local response to healthy outcomes and health inequalities.
  • Supporting the health awareness of individuals, groups, organizations, and empowering them to make healthy choices related to chronic disease prevention such as tobacco use prevention/cessation, healthy eating, and active living.
  • Leading and supporting multi-agency projects to promote a healthy environment.
  • Researching, developing, and implementing evidence and population-based interventions which support health promotion and chronic disease risk reduction strategies.
  • Ensuring attendance at required trainings and quality assurance activities as needed.
  • Updating and maintaining skills, information, and knowledge by participating in community events, seminars, and webinars.
Minimum Qualifications and Requirements
  • Bachelor's degree in Social Science or related field.
  • Two (2) - Three (3) years of community-based work experience in community health, community outreach, or social services.
Community Health Advocate I: Initiation/completion of Community Health Worker (CHW) educational requirements within one year of employment. Community Health Advocate II: Community Health Worker Certification (CHW)
  • Proficient with Microsoft Word, Excel, and PowerPoint.
  • Understands and acknowledges that Public Health - Dayton & Montgomery County uses evidence-based and evidence-informed public health practices based on scientific research including health promotion theory and practice, environmental health measures, and communicable disease control measures including vaccination, isolation, and quarantine practices, and medicine for treatment and prevention of disease, among others.
  • Understands and acknowledges the use of tobacco or nicotine products in any form including replacement therapy products and prescription medications used to treat smoking addiction for anyone hired after January 1, 2014, is prohibited.
  • Required to play an active role during a public health emergency, crisis, outbreak, incident, or event in which the Health Commissioner deems additional personnel resources are warranted. This may include changes in responsibilities and working locations/hours.
  • Completion of Incident Command System 100 and National Incident Management System 700 courses within 90 days of hire.
  • Valid State of Ohio driver's license and insurance on any personal vehicle that will be used for work.

  • Master's degree in Social Science or related field
  • Community Health Worker Certification (CHW)

Seasonal employees scheduled to work 40 hours during their period of employment will accrue vacation in the same manner as regular full-time employees and can use their vacation after completion of twenty-six (26) bi-weekly pay periods in active pay status. Part-time Seasonal employees (scheduled to work less than 40 hours) do not accrue vacation. Intermittent and Temporary Employees: Long-Term Sick Leave accrual is based on hours worked.
