Community Habilitation Specialist (Mon - Fri, 8:00 AM - 3:00 PM) - Mercy Home for Children : Job Details

Under the general direction of the Coordinator of Support Programs, the Community Habilitation Support Professional is responsible for providing services to the individuals consistent with the established Individual Service Plan as well as the In-Home Residential Habilitation Plans.
