Are you passionate about helping people transition from institutional care back into the community? Join our mission-driven team as a Community Care Manager to support individuals on their journey toward independence and wellness.

What You'll Do :

As a Community Care Manager, you'll work one-on-one with clients transitioning from nursing homes and long-term care facilities. You'll create and manage personalized care plans, coordinate services, attend appointments, advocate for client needs, and provide hands-on support, from housing searches to setting up SNAP benefits. Every day is different, meaningful, and impactful.

Location : Based in Lombard, IL, with travel across the Chicago metro area and nearby suburbs.

Requirements : QUALIFICATIONS

What You'll Need :

  • Master's in Social Work, Counseling, Psychology, or a related field
  • 3+ years of experience supporting individuals with mental health, substance use, or complex medical needs
  • Valid driver's license & reliable vehicle
  • Strong communication, advocacy, and organizational skills
  • Passion for empowering others

Compensation & Benefits :

  • Base Salary : $60,000 per year
  • Paid Time Off : 11 Vacation days, 12 Holidays days, 3 sick days, 3 personal days
  • Illinois Paid Leave : 40 hours (accrued)
