The Virtual Nurse is responsible to provide and coordinate care to individuals and families using a holistic approach. The nursing process is used as a structured mechanism for critical thinking and problem solving in providing effective and appropriate care within a safe and confidential environment. The incumbent is responsible for collaborating with members of the healthcare team, personal/professional development, and conducting him/herself professionally. The incumbent will have excellent communication skills, with an overall understanding of the continuum of care needs, safety, quality and patient experience outcomes. The incumbent will be able to effectively and efficiently speak with patients to facilitate progression of care, including education, assessment clinical status and providing interventions that are patient specific partnered with patient/family.
