Job Description
Responsible for coding and analysis of complex inpatient medical records, utilizing ICD-9-CM and ICD-10-CM coding guidelines and conventions for establishing diagnoses and procedures. The coded information supports reimbursement, data retrieval, education, medical research, clinical care assessment, and public health monitoring.
Responsibilities
- Review, analyze, and interpret medical records (electronic and hardcopy) to identify all diagnoses and procedures affecting the current admission.
- Apply knowledge of anatomy, physiology, disease processes, and pharmacology to identify diagnoses and procedures.
- Translate diagnoses and procedures into accurate codes using ICD-9-CM and ICD-10-CM, adhering to official coding guidelines.
- Abstract information for reporting to the Congenital Malformations Registry (CMR) as mandated by New York State.
- Utilize resources such as AHA Coding Clinic, coding handbooks, and electronic references to ensure accurate coding and reimbursement.
- Notify Coding Compliance Managers when documentation clarification is needed.
- Use encoder logic, grouper software, and clinical analyzers to assign codes and DRGs.
- Maintain and update coding skills through continuous education, keeping pace with medical advances.
- Comply with federal and state regulations, and adhere to AHIMA's ethical coding standards.
- Maintain confidentiality of all patient records and information.
Qualifications
- High School Diploma or GED
- Certified Coding Specialist (CCS) credential required
- Thorough knowledge of Medical Terminology, ICD-9-CM, ICD-10-CM, and DRG logic.
- Experience in Medical Records or related field; 2+ years hospital coding experience preferred.
