Job Summary: A member of patient care team within the Department of Home Health and directly accountable to Patient Clinical Manager. The position requires that the individual assist the professional staff by performing specific nurse aide functions related to patient care to include bathing, shampooing, ambulation, transfers, tidy sick area, skin assessment, and other duties as identified on care plan under the supervision of a registered nurse or therapist. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The CNA prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies RN/Therapists of any changes in patient and assists in meeting needs. CNA participates in in-service and competency programs as applicable. Performs other job duties as assigned.
