Clinician – Youth & Family Services (PACT-Y Program) - The Home for Little Wanderers : Job Details

Collaborate within a multidisciplinary team to support youth in achieving their recovery goals (maximum caseload of 6). Conduct strength-based assessments, develop individualized treatment and safety plans in partnership with youth, families, and team members. Deliver evidence-based clinical interventions, including family therapy, crisis intervention, and parent education. Provide in-home and community-based therapeutic services to stabilize and support youth and families. Maintain accurate and timely documentation, including progress notes and program-required reports. Participate in an on-call rotation for crisis response. Conduct substance use assessments and provide appropriate interventions as needed. Qualifications Master's Degree in Psychology, Counseling, Social Work, Family Therapy, or a related field. Licensure preferred (LMHC, LICSW, LMFT) or license-eligible.
