We are currently seeking a licensed Clinician to join our OnTrack team, in the role of Primary Clinician (PC). The role of a PC is an exciting opportunity for a clinician to utilize creative and out-of-the-box strategies that will expand on the traditional approaches for care, to help engage and support young people experiencing their first episode of psychosis.
In this role, you will work as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of serious mental illness. This role combines clinical responsibilities with community engagement and recruitment. The ideal candidate will serve as the primary clinician for a subset of participants while also leading outreach efforts and managing referral relationships.
This is a full-time position with a salary range of $65,000 - $75,000.
Responsibilities:
As a valued member of our team, your responsibilities will include, but are not limited to:
Serving as the primary Clinician for a subset of program participants.
Conducting outreach and recruitment through community education and presentations.
Serving as the primary contact for referral sources.
Maintaining a database of referral sources with up-to-date contact information.
Screening referred individuals for program eligibility via phone and in-person assessments.
Making referrals to appropriate community resources, as needed.
Participating in weekly team meetings, and ongoing training and supervision.
Sharing rotating on-call responsibilities with other team members.
Requirements:
We are seeking candidates who possess the following qualifications:
Masters degree (LCSW, LMSW, LMHC) from an accredited school
NYS Licensure is required
Training and experience working with adolescents and/or young adults with serious mental illness
Experience in community-based mental health services
Proficiency in providing clinical assessments
Strong interpersonal, organizational, communication skills
Familiarity with community resources and support systems for mental health care
