What You'll Do:

Provide individual therapy and support in clients' homes, schools, or the community

Help clients access vital services and stay on top of benefits

Create and lead engaging therapy groups

Keep detailed records and stay on top of clinical documentation

Be a steady, supportive presence in your clients' lives

Essential Responsibilities, include but are not limited to:

• Conduct Mental Health Assessments and Semi-annual treatment planning for assigned clients using the State approved IM/CANS tool ensuring the treatment plans incorporate objectives that reflect the identified needs of the individuals served.
• Provide individual therapy, individual assessment and treatment planning to clients ages ranging from 18-65 years and older clients.
• Clinicians provide therapy services to individuals: in the community, at EU day centers and via telehealth.
• Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
• In conjunction with the client's treatment team, keep the family, guardian and other Envision Unlimited staff informed of the client's progress.

QUALIFICATIONS:

What You Bring:

Master's degree in a behavioral health field

3+ years' experience (bonus if it's with seniors!)

Great communication + problem-solving skills

Ability to work independently

A valid driver's license and a passion for helping others

Must be at least 21 years of age, possess a valid driver's license or state ID, be approved to drive agency vehicles through agency insurance, pass a criminal background check. Must have excellent written and oral communication skills.

• Base Salary: $55,000 per year
