The PCM RN Clinical Supervisor is responsible and accountable for organizing and directing all aspects of the PCM RN clinical team. The PCM RN Clinical Supervisor reporting directly to the PCM Manger works collaboratively with the PCM SW supervisor on daily operations of the PCM Dept, the Director of Continuum of Care and/or appropriate Chain of Command and hospital departments in supporting case management operations, care coordination, and patient throughput. The PCM RN Clinical Supervisor serves as a positive role model and sets the professional tone for their area. The PCM RN Clinical Supervisor provides leadership and management in the areas of staff customer satisfaction, provision of safe discharge plans and effective care coordination, human resources, communication, patient advocacy, performance improvement and leadership/professional development.
