What Youll Do:As a Clinical Social Worker, you will work closely with primary care teams to assess patient needs, periodically provide short-term counseling based on needs, and connect individuals with essential resources. Youll play a key role in enhancing patient well-being and improving care coordination. Key Responsibilities:?? Conduct comprehensive assessments and develop individualized care plans.?? Provide short-term counseling and crisis intervention on a very limited basis as needed.?? Advocate for patients, helping them navigate healthcare systems and community resources.?? Offer health education and promote self-management strategies.?? Collaborate with physicians, nurses, and behavioral health specialists to enhance patient Qualifications.
