The Clinical Social Worker is a professional clinical social worker whoprovidesdirect services to the patients. Primary Duties:
Providesclinical social work services toidentifiedpatients and families. This includes outpatient and inpatient services. Includes individual and group counselingregardingthe impact of illness and hospitalization. Collaborates with other health care professionals as part of a multi-disciplinary team. Facilitates communicationregardingrelevant psychosocial issues of patients and families to health careteamsthrough recording in medical records (inpatient and outpatient),consultationand participation in relevant meetings.
Functions as a coordinating link between patient, family,hospital,and community resources to assure continuity of care.Representssocial work perspective and serves as department representative todesignatedHospital and Lombardi Cancer Center committees and community organizations.
Participates in orientation, training, and education programs for students, physicians,nurses,and other health care professionals asrequired. Assumes responsibility foridentifyingtraining needs,locatingresources,training needs,locatingresources, and promoting own professional development.
Collaborates with other disciplines in development ofnew programsthat enhance the quality of services to the Oncology Patient/Lombardi Cancer Center patient and their significant others. Collaborates with Clinical Nurse Coordinator staff andassistsDischarge Planning Case Management Staff tofacilitateappropriate placementof patients. This may bepost-hospitalization or when clinical conditionswarrant.
Actively cooperates as a participant in the Social Work Department, assuming backup-up and on-call responsibilities, committee work, research, QA/QI, and other duties asappropriate.Mayprovidesupervision to LGSW-level social workers.
Qualifications:
Master's degree of Social Work from an accreditedtrainingprogram.
2years' experiencewith acute and chronically ill patients(Hospital setting preferable).
Experience with oncology patientspreferred.
