The Clinical Program Coordinator (Infection Control) is responsible for the complex, multi-factorial, multi-disciplinary care coordination and independent management in Infection Control, which includes developing, evaluating and revising care pathways based on assessments conducted through engagement with the patient, family and multiple departments and disciplines. The Clinical Program Coordinator interfaces directly with MSH Departments, Administrators, Nursing, Medical and Quality leadership, and clinical staff and coordinates care for a patient population regarding the clinical, educational and operational impact of all care delivery, and outcome measures.
The Clinical Program Coordinator is responsible for program development, which includes the independent management of a program of care for a group of patients; carried out for the duration of hospitalization and includes changing practice to meet outcomes; evaluation of practice; providing just in time education and implementing corrective action plans. The Clinical Program Coordinator is expected to flex their work schedule to manage the above.
May be accountable for the collaborative development and execution of programmatic policies, procedures and guidelines, organization-wide.
