Clinical Nurse Coordinator (MDS) - Soloh Partners : Job Details

Position: Clinical Nurse Coordinator (MDS) Location: Brooklyn, NY Schedule: 9 am to 5 pm Pay Rate: $ 68Overview: MDS Coordinator is responsible for coordination of MDS assessments of each resident. Participates in the overall process of providing optimum care of each resident. Duties:

• Completes accurate assessments, MDS, and CAAs as assigned.
• Monitors and audits clinical records, assures accuracy and timeliness.
• Informs PI Director of persistent issues related with non-compliant documentation.
• Communicates with physicians, discharge planners and others to process referrals, authorization for services
• Ensures accurate ICD-10 coding for proper reimbursement.
• Monitors Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement.
• Reviews daily all communications received via fax, email or mail.

Requirments:

• BSN - required
• Licensed Nurse in New York State
• Minimum of two (2) years of experience in MDS 3.0.
• Long Term care experience required.
• Proof of full vaccination required
