This is a 12+ Months Contract opportunity with long-term potential and is located in McAllen, Edinburg, Corpus Crist, Brownsville Weslaco TX (Remote). Pay Range: $27 - $30/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Caseloads will be 20-25 members
• Follow ups by phone and email, will have to be completed for all members making sure all referrals for each member are completed
• Follow ups will be made to doctor's office, DME, etc
• Documentation will be done through Teams channel excel sheet
• Manager is available through teams and email, first point of contact will be team lead

Key Requirements and Technology Experience:

• Skills-LSW, Case management
• Excellent computer skills
• Good Communication skills
• Organization
• Home Health experience
• Case Management experience 2-10 years
• Nursing facility experience
• Bilingual
• Bachelor's degree and 2 – 4 years of related experience
• LSW
