Clinical Intake Manager - Admissions DepartmentSkilled Nursing & Rehabilitation Full-time | Skilled Nursing Facility specializing in geriatricsAbout the Role We're looking for a detail-oriented, compassionate, and responsible Clinical Intake Manager to join our Admissions team. In this role, you'll coordinate and manage the admission process for older adults seeking both sub-acute rehabilitation and long-term care .You'll review clinical referrals, assess appropriateness for placement, and work closely with families, hospitals, and our care teams to ensure a smooth and welcoming transition for every patient and resident. What You'll Do: • Review and evaluate incoming referrals from hospitals, physicians, and community sources. • Assess clinical records to determine admission eligibility and level of care needs. • Coordinate admission scheduling, insurance verification, and pre-admission assessments. • Serve as a liaison for patients, families, and referral partners. • Maintain accurate admissions documentation and ensure compliance with regulatory standards. • Build and maintain strong relationships with referral sources to support census growth. • Collaborate with social workers, nursing, other clinicians and team members to insure seamless transitions for our geriatric population. • Serve as a resource for understanding sub-acute and long-term care programs at the Hebrew Home in Riverdale. What You Bring: • RN, LPN, or other qualified healthcare professional (Social Worker, Care Coordinator, Case Manager, Utilization Review Specialists). • Geriatrics experience preferred. • 4+ years in skilled nursing, sub-acute rehab, or hospital discharge planning. • Strong understanding of geriatric care and post-acute services. • Exceptional communication, organizational, and decision-making skills. • Proficiency with EMR systems and Microsoft Office. Key Skills: • Strong assessment and care planning abilities. • Excellent verbal and written communication skills. • Ability to interpret clinical and psychosocial needs to match with the right care level. • Proficiency in Microsoft Office and comfort using electronic referral systems. • Commitment to service excellence and regulatory compliance (NYS DOH standards).
