Clinical Documentation Specialist - CENTER FOR HUMAN DEVELOPMENT : Job Details

The Center for Human Development, (CHD) is seeking a highly experienced Clinical Documentation Specialist to join CHD's Organizational Excellence team. This is a full-time, fully onsite position located in Springfield, MA. Your role as the Clinical Documentation SpecialistThe Clinical Documentation Specialist will interface with leadership staff across the entire organization to design and implement a system to monitor the quality and integrity of clinical and medical documentation. This individual will take the lead in the design and implementation of organizational clinical documentation review system.The Specialist will:

• Conduct and oversee the auditing of records.
• Identify gaps and ensure implementation and monitoring of performance improvement.
• Collaborate closely with clinical and medical staff to review and generate performance improvement plans based upon findings resulting from clinical quality chart reviews.
• Work with the Corporate Quality Manager and other quality and compliance staff to improve the quality and completeness of systems for monitoring and auditing of clinical records.
• Work with leaders to identify documentation trends and minimize risk.
• Develop and coordinate education and training pertaining to documentation opportunities utilizing a variety of methods to deliver content, such as direct provider collaboration, power point presentations, and Teams meetings.

By conducting and overseeing the auditing of records the Specialist will ensure the identification of gaps as well as the implementation and monitoring of performance improvement. The Specialist will contribute to the delivery of high-quality healthcare services and minimize legal and financial risks, improve the quality of healthcare documentation and ultimately client care.Requirements:

• Master's degree from an accredited Social Work program or Bachelor's degree from an accredited Nursing program
• Licensed social worker or RN preferred
• Previously experience in mental health, substance abuse and medical treatment modalities and assessment tools.
• Previous experience with Electronic Health Record systems, Netsmart's myEvolve preferred.

Success Factors: The ideal candidate will have knowledge of state regulations (MassHealth, DPH, EEC, BSAS, DDS, BORIM etc.). They will leverage their experience and understanding of clinical treatment to evaluate chart/clinical information, ask appropriate questions, analyze data, and present rationale for improvement recommendations to operational, clinical, and medical professionals. The candidate must be highly organized with effective problem-solving skills, and a strong aptitude for computer functionality, specifically with the Office suite, including Excel, Word, PowerPoint, Teams and Outlook.Take advantage of a competitive salary range of $75,000-$85,000.
