Positions provide nutritional assessments, counseling and follow up. Oversees diabetes education program. Might provide any of the following based upon program, physician and department needs: develop nutrition awareness programs, provides instruction, treatment, education materials, consultations, diet analysis, cooking demonstrations. - - In Medical Home Practices (PCMH/ CPCI), this role may also work with patients of multiple chronic disease states including Diabetes, Obesity, Heart Disease, Hypertension, and well states to achieve optimal outcomes post-surgery or per consult request per physician. Job Requirements: As of 1/1/24, new practitioners are required to have a Masters Degree in Dietetics.
