Your expertise will shape discharge planning, psychoeducational programming, and group therapy experiences that leave a lasting impact. What You'll Do:Coordinate intake and pre-admission screenings. Assess patient suitability for admission through clinical judgment. Educate patients, families, and healthcare professionals on rehab options and facility expectations. Serve as liaison with external case managers, discharge planners, and payors. Conduct face-to-face educational sessions with physicians and case managers. Perform market research and implement development strategies. Gather and secure patient resource and benefit information. Apply knowledge of developmental and aging processes to ensure appropriate care delivery.
