CLINICAL CASE MANAGER - NFI North : Job Details

NFI North, Finson Road in Bangor, ME is seeking applicants to fill its Full-Time Clinical Case Manager position. Finson Road is an 8-bed residential program for older adults living with mental illness. This person will provide case management services for the program including, coordinating with external providers and funding assistance agencies, treatment team meeting involvement, group and family work, support in developing and implementing treatment goals, discharge planning, provision of aftercare and outreach, and crisis intervention. Applicants must possess a Bachelor Degree in Social Work, Psychology or a related field. LSW preferred but not required. MHRT-C certification or eligibility is required.
