Reports to: Director of Client Care

FLSA Status: Non-Exempt, Full-time (1.0 FTE)

Location: San Diego, California

Salary: $65,000 – $75,000

General Purpose of the Job: The Clinical Case Manager will perform linkage, referrals and a warm hand-off to community-based organizations such as primary care clinics, health care plans, alternative healing centers and organizations, faith- based institutions, ethnic organizations, and peer-directed programs such as Clubhouses.

Essential Duties and Responsibilities: This job requires remote work, in-office work, in-field work or a combination. (The examples of functions are not exhaustive.)

  • Meet with assigned caseload of clients on a quarterly basis to assess for and complete case management goal/plans
  • Assist clients in applying for and obtaining social and other services, including supported employment and healthcare services (e.g., Medi-Cal enrollment)
  • Connect clients to transit (e.g., San Diego Metropolitan Transit System), benefits (e.g., Social Security Administration), housing (e.g., local housing authorities), money management (e.g., payee services)
  • Build, maintain and refer to network of community resources
  • Conduct home visits and mobile outreach e.g., community-based treatment
  • Advocate for client services and professionally represent agency
  • Identify critical client care issues for clinical meetings and supervision
  • Seek guidance from and report crises to Director of Client Care
  • Facilitate one Healing Club per year
  • Document client services in SmartCare and ETO (electronic medical records) database ensuring data is complete, accurate, and contract-compliant

Minimum Qualifications:

  • Bilingual proficiency in Farsi/Dari preferred
  • Strong commitment to the mission of SURVIVORS of Torture, International
  • Bachelor's degree in social work, psychology, or related field or related combination of education/experience
  • Possess strengths-based, client-centered, trauma-informed clinical skills
  • Knowledge of client services databases such as SmartCare and ETO preferred
  • Must have reliable car, valid California Driver's license and adequate auto insurance
