Clinical Care Coordinator Unit - FT Days - Grady Health System : Job Details

The Clinical Care Coordinator (C 3) proactively identifies and removes barriers that my impede department or system wide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The position (Unit C 3) will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review to ensure appropriate bed is assigned, patient care needs are met and treatment plans executed at the appropriate levels. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization. JOB QUALIFICATIONS: Bachelor's Degree in a health related field is required. Three (3) or more years of clinical experience is required. Nurse Case Management experience is preferred.
