- Maintains an in-depth knowledge of nursing principles, practices, standards and techniques, and applies this knowledge in accordance with Sky Lakes/CEFPC policies and procedures and within laws and regulations governing nursing practice in the State of Oregon.
- Functions as a team member, implements appropriate nursing actions to meet the patient's physical, emotional, spiritual, social and intellectual needs. Functions as care manager to patients to high risk patients.
- Provides appropriate nursing assessment of patients during intake, phone triage and prioritizes interventions as necessary.
- Accurately documents information and observations that contribute to the patient's assessment and plan of care and. reports abnormalities to physician. Accurately documents education provided to patient/family.
- Assists provider or co-workers with coordination of care including patient admission to hospital and/or other facilities, schedules patient procedures and appointments as needed and calls or faxes approved orders to care homes, nursing homes or hospitals. Assists with medication refill process following CEFP guidelines.
- Administers injections/immunizations and/or other medications, start and manage IV lines, per orders and completes appropriate documentation when providing clinical care.
- Provides nursing support to clinic patients, both in person and on the phone.
- Assists medical assistants with well child visits including knowledge of immunization practices. Accurately completes documentation of immunizations and growth charts.
- Demonstrates an awareness of cost containment and economical use of supplies and equipment. Appropriately documents charges in EMR.
