Position Summary:Provides care management and care coordination for adult and pediatric patients with mild to complex illness, in the primary care setting.

Reports to: Director

FLSA: Exempt

ESSENTIAL FUNCTIONS:

  • Identifies the targeted population and implements clinical interventions and protocols.
  • Collaborates with PCP, patient and members of health care team including continuum of care settings and community resources.
  • Provides self-management support and includes patient/family as active members in the team approach.
  • Facilitates close monitoring of high risk patients to prevent and/or intervene early during acute exacerbations.
  • Coordinates care between PCP and acute care hospitals, specialists and post-acute care services.
  • Qualifications

    Minimum Qualifications:Minimum Education:Associate's degree in NursingBachelor's degree in Nursing (Preferred)*Years of relevant experience may be substituted for required education.

    Required Skills:Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

    Certifications:Basic Life Support Health Care Provider (BLS-HCP)Licenses:Registered Nurse (RN) in State of Michigan

    Minimum Work Experience:2 years in adult medicine and pediatric patients

    Full-timeWork Schedule: Day shift, 7-10 hr/shift, weekdays only
