Senior Helpers is looking for a passionate professional to fill the very important role of Client Care Coordinator. The Client Care Coordinator (CCC) will have the overall responsibility of managing our clients for a defined area within our franchise territory. The CCC will be the main point of contact with assigned clients and key responsibilities will include converting leads, retaining clients, and managing the growth of hours from current clients.Reports to: Executive Controller / DirectorJob Duties: Has a regular communication process for new clients that may include caregiver introductions, first day of service calls, and visit(s) within the first 30 to 90 days of serviceEnsures that clients and their families are satisfied with the Senior Helpers services, including online reviews, and works to find solutions with the appropriate employees and/or family members, as neededVisits clients on a systematic basis to assess the quality of services as well as the client and family satisfaction. Spends approximately two to three days out of the office reassessing clients and managing client visits in the hospital, rehabs, facilities, etc.Adapts and communicates the client care plans as neededCommunicates with scheduler or Care Team Manager if caregivers need to be changedGenerates internal growth each month consistent with company goalsCompletes assessments and reassessments using LIFE ProfileUpdates the client retention dashboard on a weekly basis and creates a plan for replacing lost hours and proactive care management weeklyTakes service inquiry calls and follows up with leads to meet client conversion targetsCover client shifts on rare occasions where a replacement is needed on an emergency basis.Other duties as assignedQualifications:Bachelor's degree and three years of related work experience preferredMinimum of 2 years experience managing clients and employeesPrevious industry experience requiredMust have exceptional organizational skills, attention to detail, and the ability to prioritize in a changing environmentMust have excellent verbal communication skills and follow-up skills with prospects, clients, client families, and referral sourcesAbility to quickly build rapportCreative and innovative problem-solving abilitiesProactively prevent and resolve issuesAbility to visit client homes in the assigned territoryProficiency in Microsoft Word, Excel, Internet, and Outlook requiredAbility to learn other software programs quicklyAbility to work independently and as part of a teamJob Benefits:Great CulturePaid Time OffShort term & Long-term DisabilityPay On DemandWeekly PayBonus structureLifeMart BenefitsWhy Work for Senior Helpers?We are changing Homecare.
