Job Location:

49 Forest Road, Monroe, NY 10950

Job Responsibility

  • Utilizes patient-centered care to help build rapport and help improve health of chronic patients
  • Complete chronic care patient lists and follow up with patient to ensure they are compliant with medications and follow up visits.
  • Completing hospitalization follow ups using the Azara
  • Diabetic care management (obtaining lab results, ensuring follow up with ophthalmology and vision, and ensure diabetic foot exam is completed/following up with podiatry)
  • Completing complicated messages from providers in regard to patient s health.
  • Completing nurse visits (injections, PPD, medication teaching, insulin teaching)
  • Working with Specialty providers as the center grows like Cardiology
  • Completing intakes/scribing, obtaining medical records, ordering diagnostic imaging, ordering heart monitors and offering education, following up on heart monitor results, ordering cardiac procedures to outside facility
  • Ordering home sleep tests, sleep medicine, and obtaining results
  • Ensuring CPAP compliance, ordering CPAP machine and supplies
  • Ordering supplies for patients via Parachute Health
  • Writing letters for providers
  • Completing phone triage for patients with emergent concerns.
  • Willing to work in a team environment
  • Energetic and willing to learn

Job Qualifications

  • RN Associates degree
  • 2+ years experience preferred
  • Care Management experience preferred

Schedule

  • 40 hours a week
  • 9am to 5pm
  • Work Wednesdays and Sundays that Cardiologist is scheduled.
  • Willing to be flexible as needed.
