Essential Functions 1. Provides transportation for clientele without transportation to local and out-of-town healthcare facilities. 2. Improves relations between healthcare providers and program clientele through home visits and community meetings. 3. Assists in planning and implementing health education workshops. 4. Monitors clients by conducting home, hospital, and nursing home visits. 5. Interprets/translates medical information to client or client caretaker from medical personnel. 6. Assists clients with arranging transportation, medication delivery, and/or medical appointments. 7. Provides client and emergency care when needed or requested (i.e., vital signs, medical equipment, and emotional support). 8. Ensures healthcare for clients in need and makes referrals, when necessary. 9. Maintains appropriate documentation on all clients. 10. Submits monthly report of activities to CHE/HE Coordinator. 11. Attends training to enhance health care knowledge and job skills. 12. Participate in CHR/HE Program, DHCS, and tribal events (when required). 13. Contributes to a team effort. 14. Performs other duties as assigned and within the scope of the position. 15. Be able to work Wellness Program evening activities (when needed). QualificationsEducationalRequirements Minimum: High school diploma or GED; Preferred: One (1) year of college/vocational coursework in health-related field. ExperienceRequirements Minimum: One (1) year ofhealth-related work experience. Preferred: Two (2) years of health-related work experience. Knowledge, Skills, & Abilities Computer:
Familiarity with Microsoft applications, including Word, Excel, Outlook, Internet Explorer, etc. Proficiency in the proper use of and operation of stand office equipment. Communication:
Ability to establish and maintain professional relationships with co-workers, medical professionals, Indian Health Service, and other outside agencies. Ability to communicate effectively with program clientele, the general public, tribal members, Pawnee Nation employees, and other concerned parties. Must possess strong administrative and communications skills. Miscellaneous:
Ability to pass a background screening. Must possess the ability to provide clients with compassionate care respective of Native American customs, traditions, and beliefs. Must demonstrate a basic understanding of clients' rights and responsibilities, including familiarity with confidentiality requirements. Work Environment Work is generally performed in an office setting with a moderate noise level. Evening and/or weekend work may be required.
