Job Description JOB SUMMARY This paragraph summarizes the general nature, level and purpose of the job. Provide diabetes education services for the Diabetes or Obstetric Programs. Collaborate with the clinical care teams in regards to appropriate diabetes management and education protocols in the inpatient and outpatient setting. Provide diabetes education classes and support group services. ESSENTIAL FUNCTIONS The essential functions listed are typical examples of work performed by positions in this job classification. Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct. *Provide diabetes self-management education (DSME) for newly diagnosed patients with diabetes and their families. *Facilitate outpatient diabetes classes pertinent to the Stanford Children's diabetes education program. *Make recommendations involving diabetes management supplies and diabetes devices, including insulin delivery devices, insulin pumps, blood glucose meters, and continuous glucose meters. Provide ongoing education for both the family and patient with diabetes. *Assist with training patients starting on insulin pumps and continuous glucose monitors. Provide instruction on use of glucometers, CGM devices, and insulin injection using insulin pens or syringes *Communicate with patients and families regarding patient care in clinic settings, via Mychart, via email, and via phone call follow up. Ok in both *Collaborate with medical and hospital nursing staff with discharge planning of patients with diabetes and other patient / family interventions as needed. *Provide requested in-service training and presentations on pediatric diabetes care for hospital staff, and outside organizations. *Assist with prior authorizations for diabetes medications and diabetes devices when medically necessary, but not covered benefit of patient's insurance *Facilitate diabetes medication prescription renewals, routed to providers in electronic medical records for review and signature. *Monitor patients' diabetes behavioral goals and outcomes' communicate and document patient progress for the healthcare team. *Assist in-patient hospital staff with diabetes management skills and education. *Interact with the multidisciplinary diabetes team in outpatient clinics that provides comprehensive diabetes care and education. *Provide outreach and support to the diabetes population, including facilitating out of Stanford system referrals to diabetes management teams (physician and staff) local to patient. Qualifications MINIMUM QUALIFICATIONS Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying. Education: Bachelor's degree in a work-related discipline/field from an accredited college or university. Experience: One (1) years of progressively responsible and directly related experience.
