Description Part-timeResponsible for obtaining essential social information from acute care patients and their patients, as well as providing counseling services as appropriate. Works in close cooperation with other health care providers, social service agencies, hospitals, clinics, and community resources in planning to meet the needs of patients, acts as patients advocate to ensure that the patient utilizing all services available. Minimum Requirements Requires a bachelor's degree in social work or related fieldPrior experience as a health care system caseworker preferred.
