Job Title: Case Worker

Location: New York, NY 10016

Shift Time: 9:00AM - 5:00PM

Duration: 8 weeks with strong possibilities of extension

Required Skills & Experience:

• No active patient caseload — Focused exclusively on nursing home placements

• One (1) year of experience.

Job Summary:

• Interviews, makes initial assessments & referrals.

• Manages cases of social service recipients.

• Makes home visits to adults to assess their needs.

• Investigates & verifies information obtained.

• Provides health related and social services to clients.

Required Education:

• Associate's Degree.

Preferred Education:

• Bachelor's Degree.
