Casework Associate

This employee performs a variety of clinical, administrative, and clerical tasks in support of a health program. Employee observes behaviors, conducts interviews, makes initial assessments, and monitors the progress of clients of programs, including direct services; performs a variety of duties as part of a team approach to health care; serves as an assistant to and under the direction of a Team Leader. Performs related duties as required.

Minimum Qualifications (KSAs) (Illustrative only. Any one position may not require all of the listed KSAs nor do the listed examples include all the KSAs which may be required.) Knowledge of: Basic business English including spelling, punctuation, grammar, and sentence structure. Standard office methods and practices. Interviewing techniques. Available social service resources. Electronic medical record (EPIC) Skill to: Properly operate a motor vehicle. Ability to: Write clearly, concisely, and logically. Adapt to a wide variety of duties. Follow oral and written instructions. Establish and maintain effective working relationships with others. Maintain confidentiality of client information. Make sound judgments based in logical analysis. Communicate tactfully and effectively, both orally and in writing. Work without close supervision. Interact effectively with withdrawn and hostile clients. Be flexible in dealing with alternative life styles among clients. Required Education and Experience RESUME, TRANSCRIPTS, AND CERTIFICATION MUST BE ATTACHED OR APPLICATION WILL BE REJECTED Each applicant must have two years of paid work experience in social services, education, medical/ health office, national and community service, recreation, or a closely related field with the major emphasis of that experience on interaction with children and families OR Must have 90 quarter credit hours (60 semester hours) in college courses in any combination of the following fields: English Composition, Education, Health Education, Social Sciences, Psychology, Nursing, or related field OR must have a combination of experience and education equal to two years. (45 quarter hours = 1 year). OR State of Ohio Community Health Worker Certification. Related experience may be substituted for education on a year-to-year basis.

Other requirements Some positions may require a valid Driver's License. Must be willing to work irregular hours and at various locations. Bilingual candidates preferred.

Examples of Work Performed: (Illustrative only. Any one position within this classification may not include all of the duties listed nor do the listed examples include all of the tasks which may be performed.) Conducts outreach activities in geographic areas where clients can be reached; these activities include distributing brochures and information about health care and social services available in the community. Assists in collecting information about a client including physical, psychological, social, medical, and the client's capacities through interviews and intervention sessions. Assists in the establishment of intervention goals for the client based upon the assessment process. Assists clients regarding their follow-up in the treatment plan under the direction of a Team Leader. Maintains client files, program files, therapy equipment, and materials. Writes reports. Assists clients in obtaining basic necessities (food, shelter, transportation, etc.) Drives clients to the hospital or other health care providers by car or department vehicle. Drives to and from various assignments. Acts as a social service liaison to other health care agencies. Assists in planning and conducting workshops for clients, their families, and permanent contact. Maintains adequate stock of office supplies. Manages patient flow and referrals from Nurse Practitioner to Registered Nurse. Documents and completed medical records. Coordinates client referrals and follow-up appointments. Provides community groups with information concerning social services and counseling activities. Conducts follow-up sessions with clients to determine if the client kept their appointments. Attends conferences, workshops, seminars, and in-service training meetings.
