You will be responsible for conducting interviews, performing initial assessments, and making appropriate referrals to support services. Job Description
- Conduct assessments of patients' social, emotional, and medical needs.
- Develop and implement individualized care plans, ensuring follow-up and continuity of care.
- Connect patients with community resources such as housing, food assistance, mental health services, and public benefits programs.
- Maintain accurate and timely documentation of case notes, client interactions, and progress reports in compliance with HIPAA and organizational policies.
- Collaborate with interdisciplinary teams, including healthcare providers, social workers, and administrative staff.
- Provide crisis intervention and support for patients experiencing urgent situations.
- Educate patients and families on available services and resources.
- Participate in staff meetings, training sessions, and continuous professional development.
Skills Required
- Minimum of 3 years' experience in social services, case management, or healthcare support.
- Strong documentation and reporting skills, with attention to detail.
- Excellent interpersonal, communication, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team
- Experience in social services
Education/Training/Certifications
- Minimum Degree Required: Bachelor's Degree
