Case Worker - GOOD SHEPHERD SERVICES : Job Details

The Family Counseling for School Success Program provides counseling and case management services to families connected with PS/ MS 15, PS 291, PS 246 or live within the community. The Caseworker will provide strength-based counseling and case management services to assigned families utilizing Motivational Interviewing and Solution-Based Casework (SBC) Model. Case Practice Responsibilities: Conducts outreach to all assigned referrals within 48 hours of assignment. Schedules to meet with families within 7 days to assess for services. Provide ongoing individual and family counseling to children and families. Provide short-term crisis intervention. Conduct home visits. Collaborate with the family worker to provide concrete needs assessment and referrals when indicated. Creates a productive partnership with contracted schools on families involved in counseling.
