Case Manager - Support Teams for Addiction Recovery (STAR)

Join our team at Oaks Integrated Care as a Case Manager - Support Teams for Addiction Recovery (STAR). This role is based in Cumberland County and involves working in an addiction-focused program.

The STAR Program supports adults in Burlington and Cumberland Counties facing issues such as homelessness, incarceration, legal challenges, employment, education, transportation, social services, healthcare, child welfare, and treatment linkage including Medication Assisted Treatment (MAT). Our goal is to help individuals stay in recovery, reduce recurrence of opioid-related problems, and prevent overdose. Services are available 24/7 via telephone, and support is provided for up to 12 months.

Schedule: Monday to Friday, with varied shifts and two evenings per week.

Responsibilities
  • Conduct comprehensive assessments covering housing, finances, transportation, legal, employment, healthcare, and family needs.
  • Ensure clients are informed about and can access medical, treatment, and support services.
  • Create person-centered Integrated Family Case Plans with recovery strategies, priorities, and resources.
  • Train clients on relapse triggers and coping strategies.
  • Connect clients with community resources addressing HIV/AIDS, mental health, health issues, and criminal justice involvement.
  • Educate clients on HIV and Hepatitis.
  • Facilitate assessments and opportunities for skills development, education, employment, and career growth.
  • Document weekly visits and client participation, tailoring services to individual needs.
  • Motivate clients to voluntarily access health and recovery services.
  • Refer and assist clients in substance use disorder treatment and ensure continuity of care.
  • Coordinate interventions across multiple agencies.
  • Advocate holistically for clients.

    Qualifications
    • Bachelor's degree with CADC, LCADC, or Master's in health, psychology, counseling, or related fields.
    • Minimum 3 years' experience with high-needs individuals with substance use disorders.
    • Valid driver's license in good standing.
