Job Description :
Case Manager Supervisor

Join The Night Ministry as a Case Manager Supervisor. Lead with purpose and guide a dynamic team bringing care to Chicago's unhoused communities.

The Night Ministry is a Chicago-based nonprofit that provides human connection, housing, and health care to individuals struggling with housing stability or poverty. Our work is centered around two core service areas: Outreach & Healthcare and Youth Housing. Through our Outreach & Health programs, we meet people where they are, on CTA platforms, in encampments, at libraries, and across city neighborhoods; offering consistent, compassionate care and vital health services. Our Youth Housing programs provide shelter and supportive services to young adults, helping them build stability and work toward their goals in safe, affirming environments. With a history rooted in dignity, inclusion, and trust, The Night Ministry is committed to walking alongside those we serve with empathy and respect.

If you're passionate about creating meaningful change through relationship-building and practical support, we'd love to meet you.

What You'll Do

This role balances hands-on case management with team leadership and supervision. You'll help shape the service delivery strategy while ensuring consistent, high-quality support for clients across outreach programs.

Your work will include:
  • Providing direct case management services to clients and supporting staff follow-up on referrals.
  • Supervising a team of Outreach & Health Ministry case managers, including onboarding, coaching, scheduling, and conducting performance evaluations.
  • Ensuring compliance with internal and external documentation standards using EHR, HMIS, and ETO systems.
  • Building and maintaining relationships with external referral partners in housing, health, mental health, and addiction services.
  • Participating in staff and agency meetings, trainings, community needs assessments, and public-facing events.
  • Supporting program logistics, including volunteer engagement, team planning, and coordination with outreach schedules.
  • Driving outreach vehicles as needed and maintaining a visible presence in field-based work.
What We're Looking For

You're a skilled case manager, with experience in Chicago housing case management, and thoughtful team leader who brings cultural humility, clarity, and consistency to your work. You're able to respond calmly in challenging situations and guide others with integrity and compassion.

Qualifications:
  • Bachelor's degree in social services or a related field.
  • 25 years of experience in housing-focused case management.
  • Experience supervising or mentoring staff in direct service settings.
  • Valid driver's license, clean driving record, and access to a vehicle.
  • Ability to work outdoors and during varied hours (day, evening, weekends).
  • Strong communication and documentation skills.
  • Familiarity with harm reduction, trauma-informed care, and culturally competent practices.
Bonus if you:
  • Are bilingual in Spanish and English.
  • Are familiar with local referral sources and community-based services.
Why Work With Us
  • Be part of a mission-driven team making an immediate difference in people's lives.
  • Thrive in a collaborative, inclusive environment that values your experience and perspective.
  • Gain meaningful connections through your work while advocating for equity and access across Chicago.
