Job DescriptionSUMMARY The Case Manager is responsible for supporting CTCLUSI Tribal members by delivering comprehensive community and social services that promote individual well-being and long-term stability, including access to safe and secure housing. This role is also responsible for managing grants and administering programs related to Tribal services. Additionally, the Case Manager provides education and guidance on a variety of topics such as child care, parenting, employment readiness, and healthy living, with the goal of empowering Tribal members and strengthening families.PRINCIPAL ACTIVITIES & RESPONSIBILITIESReceives and responds to referrals for at-risk Tribal families.Assesses clients' housing needs, financial status, and overall readiness for housing stability.Develops, implements and monitors individualized case plans addressing the safety, health, education, housing, and emotional well-being of families and individuals.Assists Tribal members in accessing appropriate local, state, Tribal services and programs.Provides transportation for clients as needed to support service access and engagement.Actively participates in internal staff meetings and collaborative interdepartmental planning sessions.Delivers life skills education and connect clients with relevant community-based resources and supports.Conducts home visits to assess client needs and potential barriers, offering ongoing coaching and support.Makes referrals to services aimed at improving housing retention, stability, quality of life, and client self-sufficiency.Manages an active caseload while ensuring accurate and up-to-date client records and documentation.Maintains the highest level of confidentiality in all client interactions and records.Supports required internal and external reporting needs, including documentation for grants and other funding sources.Collaborates efficiently and effectively while consistently demonstrating professionalism and maintaining positive, respectful relationships with internal teams, external partners, and Tribal members.Other duties as directed by management.LEVEL OF AUTHORITY & RESTRICTIONSNo supervisory responsibilities or decision-making authority.PHYSICAL & MENTAL DEMANDSMust be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.Vision abilities required by this job include close vision and the ability to adjust focus.May be required to push, pull, lift, and/or carry up to 40 pounds.WORKING CONDITIONS & ENVIRONMENTModerate noise level with frequent interruptions and distractions.Ability and willingness to work a flexible schedule including some evenings and/or weekends.Ability and willingness to travel locally and state-wide as needed.JOB LOCATIONTribal Administrative Office1245 Fulton AvenueCoos Bay, Oregon 97420Apply online @ www.ctclusi.orgOpen until FilledExperience and SkillsMINIMUM JOB REQUIREMENTSMust be at least 25 years of age per Vehicle Use policy.High School Diploma or equivalent.Minimum of one (1) year of relevant experience demonstrating the necessary skills and competencies required for success in this role.Must have a valid driver's license and be able to be insured to drive Tribal Government vehicles.Experience and proficiency in the use of Microsoft products (Excel, Outlook, PowerPoint, Word, etc).Ability to communicate clearly and effectively in English, verbally, in writing or by other acceptable means.This is a safety sensitive position and will be subject to pre-employment drug testing and criminal history background check, which will include fingerprinting.This position will be subject to pre-employment drug testing and criminal history background check, which will include fingerprinting.Must have employment eligibility in the U.S.Indian preference will be observed in the hiring process.
