The Case Manager serving Sherburne and Wright Counties provides targeted, person-centered support to help participants break the cycle of crisis and achieve self-sufficiency. This includes case management services through the Pathway of Hope (POH) initiative, Rapid Rehousing (RRH), and other social service programs. Key responsibilities include participant recruitment, service planning, progress monitoring, landlord and community engagement, and ensuring program compliance.
