Case Manager Physical Therapy (Full Time, Southside Home Health, Farmville) - Centra Health : Job Details

The Home Health Physical Therapy Case Manager evaluates, develops a plan of care and treats patients to maintain or restore patients to their maximum functional performance level and completes discharge planning. Provides supervision for Licensed Physical Therapist Assistants and Certified Nursing Assistants. Implements action plans to achieve or exceed service excellence benchmarks. Evaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelines. Administers physical therapy treatments appropriate to diagnosis and to APTA guidelines. Appropriately documents and synchronizes patient care according to federal, state, and local regulations. Meets productivity standards. Uses resources (e.g., time, people, supplies, etc.) cost effectively. Supervises Licensed Physical Therapy Assistants, Certified Nursing Assistants and students.
