Colonial Behavioral Health serves the citizens of James City County, City of Poquoson, City of Williamsburg, and York County.We are currently recruiting for a Case Manager (Housing) position within our Permanent Supportive Housing program. This position will provide comprehensive housing support services to individuals with serious mental illness who may have other co-occurring substance use or developmental disordersJob Duties Job Responsibilities Works directly with individuals with serious mental illness and/or serious mental illness with co-occurring substance use and/or developmental disabilities who are homeless or at risk of becoming homeless.Responsible for identifying housing needs, assist them with applying for and securing housing, and assist participants to develop and implement a housing plan designed to assist the participant to maintain stable housing. Responsible for managing a caseload, provide supportive counseling and in-home housing focused skill supports.Other duties as assigned Work Schedule Monday - Friday 8:30am - 5:00pmOccasional evening or weekend hours based on program needs.
