Case Manager/Outreach Specialist
Department: Older Adult Services – Home-Based Services
Compensation: $30.22 – $34.07 hourly
Hours & Schedule: Full-time, Year-to-Year Contract Renewal
Position Overview
Queens Community House (QCH) Older Adult Services Department is seeking a Case Manager/Outreach Specialist to join our Homebound Services division. In this role, you will help older adults live safely and independently in their homes through compassionate care, proactive outreach, and strong community engagement.
We are looking for an organized, empathetic, and outgoing professional who thrives on building relationships and promoting services that enhance the quality of life for older adults.
Key ResponsibilitiesAssessment and Care Planning
Conduct in-home assessments to evaluate client needs, strengths, and challenges.
Develop and regularly update individualized care plans to ensure client independence and well-being.
Provide accurate information on services, benefits, and entitlements.
Coordinate services within QCH and with partner organizations, ensuring seamless support.
Maintain complete and accurate case records in accordance with program standards.
Outreach and Engagement
Visit clients in their homes to build trust and encourage participation in services.
Promote services to community groups, stakeholders, and partner organizations.
Deliver presentations that highlight program benefits and impact.
Represent QCH with professionalism and approachability at outreach events.
QualificationsRequired:
BA/BSW in Social Work, Psychology, or a related social science field.
2+ years of experience working with older adults in a center or similar setting.
Excellent interpersonal, written, and verbal communication skills.
Ability to work independently and collaboratively with diverse teams.
Strong commitment to confidentiality and professional discretion.
Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
Preferred:
Bilingual in Spanish, Russian, Cantonese, Mandarin, or Bengali.
Experience with Department for the Aging (DFTA)-funded programs.
Knowledge of local, state, and federal benefits/entitlements for older adults.
Experience using databases and case management systems.
