Case Manager, Mercer County - Social Work PRN : Job Details

Description Case Manager needed in Mercer County, NJ for a full-time, direct hire position working on-site. Responsibilities and Expectations Will have a primary caseload of 9 youths. Conducting a minimum of 2 hours of case management per youth per week, including individual sessions and group activities. Additionally, will support with intakes/discharges and complete assessments/screenings per program contract. The social worker will attend and support APN meetings, treatment team meetings, weekly case consultation, case management huddles, staff meetings, roundtables and provide support to the part-time Case Manager as needed. In additon, the social worker will manage the program store (with budget oversight from the Program Director), and participate in the on-call emergency rotation.Schedule Options (consistent weekly schedule required, flexible by semester):

Monday-Friday: Two late days (12-8 or 11-7), remaining days 9-5 or 10-6.

Sunday-Thursday: Same hours as above with only one late day.

Tuesday-Saturday: Same as Option 2.

Position Requirements Requires Bachelor's degree in social work and at least 3 years experience working with youth and adolescence. Mmust be comfortable around LGBTQIA+ youth. This position is currently accepting applications.
