A social services organization in New York is currently seeking a Case Manager to join their team. This position involves 100% fieldwork across the Bronx and Manhattan boroughs.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm (35-hour work week)
Responsibilities:
- Providing direct client services, including intake and assessment
- Managing a caseload and documenting progress in APSNET and ETO
- Monitoring risk and adapting service plans as needed
- Referring clients to appropriate external services
- Conducting home visits and telephonic communication weekly
Qualifications:
- 1+ year of Case Management experience
- Bachelor's Degree in Social Work and/or a related field
- Proficiency in Microsoft Office and standard business technology
- Solid assessment and documentation skills
Desired Skills:
- 4+ years of Case Management experience
- Bilingual (English and Spanish)
